Occupational Therapist (OT) or Certified Occupational Therapy Assistant - Skilled Nursing Facility

Horizon Healthcare Staffing has an excellent job opportunity for an Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) in a Brooklyn, NY based Skilled Nursing Facility.

We are seeking dedicated and hardworking candidates to fill our client’s needs. Occupational therapy services help patients regain functional mobility and relearn everyday physical skills that were lost due to physical, psychological, or developmental disabilities.

OT/COTA Job Details:

  • Schedule: Monday – Friday
  • Hours: 8am-4pm
  • Position Type: Full-time, ongoing
  • Location: Brooklyn, NY
  • Start Date: ASAP
  • Salary (COTA): $31.00 - $38.00 per hour
  • Salary (OT): $45.00 - $55.00 per hour

Responsibilities

Occupational Therapist (OT) Responsibilities:

  • Providing skilled occupational therapy services/interventions in accordance with Physician orders
  • Assessing patient needs, planning for, evaluating, and modifying care to meet goals of Occupational Therapy Interventions
  • Collaborating with all disciplines to plan and evaluate team goals for each patient
  • Instructing patient/family/caregivers on patient’s total occupational therapy program
  • Advising, consulting, and instructing members of the multi-disciplinary team in specific occupational therapy modalities
  • Observing and reporting patient progress and/or changes in patient status to Physician and appropriate multi-disciplinary team members
  • Other duties as assigned

Certified Occupational Therapist Assistant (COTA) Responsibilities:

  • Collaborate with the Occupational Therapist to assess residents' functional abilities, limitations, and goals, and assist in developing individualized treatment plans.
  • Implement prescribed occupational therapy interventions under the supervision and guidance of the Occupational Therapist to promote residents' independence and enhance their performance in activities of daily living (ADLs).
  • Conduct therapeutic activities and exercises that focus on improving residents' fine motor skills, coordination, strength, and endurance.
  • Assist residents in practicing adaptive techniques and using assistive devices to facilitate independence in self-care, mobility, and functional tasks.
  • Educate residents, family members, and caregivers on safe and effective strategies for improving functional abilities and promoting independence at home and in the community.
  • Monitor and document residents' progress, modifications in treatment plans, and outcomes to ensure effective care and compliance with regulatory standards.
  • Collaborate with the interdisciplinary healthcare team, including Occupational Therapists, Physical Therapists, Speech-Language Pathologists, nurses, and physicians, to provide coordinated and holistic resident care.
  • Ensure the therapy area and equipment are clean, organized, and in proper working condition, adhering to infection control protocols.

Qualifications

Job Requirements:

  • Good organizational skills
  • Good written skills
  • Licensed OT or COTA valid in New York
  • New grads encouraged to apply
