Home Infusion Pharmacist - Brooklyn, NY
Horizon Healthcare Staffing is looking for a Home Infusion Pharmacist to help operations & assist in obtaining national accreditation. The position is Monday thru Friday 9:00am to 5:00pm (must be flexible on the hours), and on-call rotation. The pharmacist will be the PIC. Salary will be determined based on experience. The pharmacist must be highly knowledgeable and experienced in compounding infusion drugs (hazardous and non-hazardous) and specialty drugs for at least a minimum of 3 to 5 years.

Schedule: Monday – Friday 9am-5pm (must be flexible on hours) and on-call rotation.
Job Duties:
- Will direct, coordinate and supervise all pharmacy activities including drug preparation such as sterile products, dispensing and clinical pharmacy services.
- Work alongside with the Director of Nursing.
- Ensure adherence to all company policy, procedures and regulations.
- Maintain schedules and on-call schedules
- Prepare reports
- Ensure that all pharmacy operations, practices and procedures meet all licensing and standards requirements established by local, state and federal agencies including accrediting bodies such as ACHC and JCAHO.
- Inspect the acquirement of drugs, supplies, and equipment used in preparing and administering drugs.
Requirements:
- A Bachelor of Science in pharmacy (Pharm D preferred) from an accredited institution with a minimum of 3-5 years pharmacy operations and clinical practice experience in home infusion therapy
- Currently licensed as a registered pharmacist in New York State
- Extensive knowledge of USP 797 / 800 standards of policy and procedure is required, and must ensure the pharmacy clean room suite meets requirements
- Pharmacist license cannot be under investigation nor have any adverse action pending from a State Board or licensing/certification agency
- Must have a strong knowledge base in intravenous admixture including mixing oncology infusions and clinical aspects of home infusion therapy
- Advanced knowledge of the practices, drug/solutions and delivery systems, drug interactions and incompatibilities, and current laws and regulations which affect the practice of pharmacy
- Ability to identify issues and problems, manage drug shortages and proactively implement strategies and resolutions in a logical and timely manner
- Candidate must be articulate and possess excellent verbal/written and listening communication skills
- Participate in on-call rotation as indicated by staffing and business needs
- Will also be interviewing, hiring, training, planning, assigning, and directing work
- Pharmacy operations and clinical practice experience in home infusion therapy: 3-5 years
