Community Transition Program RN Job in Albany
Registered nurse will work as part of the clinical support team to assist the Community Support Transition Coordinators and Operations Manager to examine and resolve nursing home and adult home clinical and operations concerns which could impede community transition for residents. The nurse shall have a strong working knowledge of nursing home admission, transfer and discharge requirements, adult care facility regulations, home care and community housing and program opportunities necessary to assist the provider with concerns affecting the transition to community settings.
Duties and Responsibilities:
- Review UAS-NY instrument and Assessment Report of individuals ineligible to transitions
- Recommend higher level of care for individuals whose medical and/or behavioral needs exceed services that can be provided in the community
- Point of contact for HHs, MLTC Plan Assessors and Care Managers and Adult Home Discharge Planners
- Receive, coordinate and manage incoming calls from residents, family members and guardians
- Perform QA review of completed Adult Home Resident Assessment Reports and generate quarterly report for Program Manager
Education:
Registered Nurse with Home Care or Discharge Planning or Adult Home Experience required
Licensed in NYS
