Description
Horizon Healthcare Staffing, a NJ leader in School Nursing Services to public and private schools, is seeking a Licensed Practical Nurse (LPN) to add to our internal staff (Monday through Friday normal business hours) in our Manalapan, NJ office.
Job Type: On Going, Temporary
Assignment: Monday – Friday (flexible - subject to change)
Starting ASAP
Responsibilities
The Intake Nurse will be responsible for:
- Processing all assigned incoming intake referrals.
- Ensuring all referrals are clinically complete, efficient, and accurate.
- Receiving job orders/contact forms for correspondence tracking.
- Contacting Case Manager assigned to client/student of the job order for questions or clarification.
- Contacting school district listed on request form to confirm information and request to conduct an intake (assessment).
- Calling parent/guardian to complete intake assessment.
- Requesting MD orders.
- Sending parent/guardian HIPPA consents and parent packets with Horizon’s information.
- Following up with parent/guardian if information needed has not yet been received to proceed with the case.
- Following up with various school districts.
- Ensuring open communication
- Addressing clinical issues as needed.
Qualifications
Qualifications:
- NJ Licensed Practical Nurse (LPN) License.
- Minimum year of clinical experience.
- Excellent phone etiquette.
- Outstanding verbal and written communication skills.
- Exceptional computer skills including Microsoft Office.
